The Lagos State Government on Saturday released an online COVID-19 test request form.
The Commissioner for Health, Prof. Akin Abayomi, disclosed this via his verified Twitter handle @ProfAkinAbayomi.
He said:
“This is to inform the general public that in a bid to make our response more effective, we are putting out forms to widen our coverage as we track down #COVID19.“Provide accurate information as required in the form with the link: COVID-19 Test Request Form”
Meanwhile, Nigeria’s confirmed cases of coronavirus infections have jumped to 209, with Lagos leading with 109 cases and one COVID-19-related death in the state.
